GENERAL APPOINTMENT INFORMATION

New Patients:
Please arrive 15-30 minutes early. Bring your insurance card and be prepared to pay your copay at check-in. Self-pay patients are required to pay at check-in. Please see Self-Pay policies.

You will need to fill out a number of papers. Please print, fill out, and bring these forms with you to decrease your wait time and ensure that we have accurate information.

It is very important that new patients bring their records with them if applicable. We cannot request these records on your behalf in most cases. You will need a HIPAA release form which needs to be completed and presented to the doctor who maintains your records. Please be aware that not all doctors offices will be able to mail your records to us in time for your appointment. Please obtain your records before your scheduled appointment to ensure that we have them atleast 48 hours prior.

Follow-up Patients:
You may be asked to verify your address and insurance information at every visit, so please alway bring and be willing to provide this information.

If there have been significant changes to your medical condition please print, fill out, and bring an update form with you to decrease your wait time.
